We are very aware of today’s
health care costs and make every effort to keep our fees fair and reasonable.
If we participate with your insurance company, we will file a claim
to them for the services that we provide to you. You are responsible for
any co-payment and/or deductible at the time of your visit.
If we do not participate with your insurance company, we expect payment
up front, before our services are provided. Our experience is that this
method works best for both parties. As a courtesy to you, we will file
a claim with your insurance company so that they may reimburse you for
our services.
Since every employer (and the designated insurance policy on coverage)
is unique, we cannot be knowledgeable on what services are covered (or
not covered) and what referrals are required. It is YOUR responsibility
to find out this information BEFORE your visit.
